Northwest Community Healthcare is conducting a Community Health Needs Assessment to identify major health problems, gaps in service and other factors that affect the health of the residents in our community. When the assessment is complete, we will develop strategies and work collaboratively with others to improve the health of our community. As part of the assessment, we are asking community members to participate in our online Community Health Survey. The link is open February 12-22, 2021.
